Transgendered, Yes; Male, obviously no.
 
A few months after the column was published, TS-Si received a comment from a person who identifies as a transman (female to male HBS man) even though his choice of an email identity, trannyboy, would suggest he is only transgendered. For the purposes of discussion, a brief excerpt from his comment follows:
I apologize if this offends you but you should be basing your opinions on the truth in this and all matters. I am a FTM transsexual, that is my label …
 
First this is not the first case of FTM pregnancy, though I agree it is unusual. I personally know of 4 cases including Thomas's. …
 
I am also aware of many FTMs who would be very insulted to hear that because they choose to keep their organs for personal reasons they wouldn't be considered FTM. I choose to get rid of mine first because that was the surgery I could get but not everyone is me. There will always be men who don't choose surgery because they feel the risks are too high …
 
I know nothing of the man's process of transitioning but I do know that he has stated his identity and you and I must respect it if we wish others to respect us … Firstly that he has not yet had surgery doesn't mean anything to his identity. His wife can't have anymore children (I believe she has one). The couple decided jointly that they wanted to have their flesh and blood and wanted another. He is not taking hormones while pregnant but has stated he will resume them after recovery from delivery and will present as the father of this child with his wife as a mother …

If HBS men and women (fna transsexual) can keep their sexual organs present at birth, the category does not exist and Dr. Blanchard is right: HBS is an extreme form of cross-dressing/transvestism. If HBS is not a birth condition that misaligns innate neurobiological makeup and external genitalia that must be corrected by surgery that brings mind and body into alignment, HBS is nothing more than glorified transgendered crossdressers with a good public relations.
 
HBS women would be what the critics charge them to be: men in dresses; HBS men (aka transmen) are women in pants, modern day Katherine Hepburns high on testosterone. Once we remove the necessity of outward physical sex matching the brain sex of the individual, what was formerly known as HBS/transsexuality becomes a psychological condition, a small subset for all intents and purposes indistinguishable from transgender.
 
Puzzle me this: if the overriding need for bottom surgery is no longer a critical prescriptive of HBS, if the desire for Sex Reassignment Surgery (SRS) is now optional, how would society tell the HBS/transsexuals from the crossdressers? How would we tell among ourselves?
 
Are HBS/transsexuals some special case that we can be taken at our word? We all can lie just as well as any crossdresser who is being placed under arrest. Or as a crossdresser, when caught en femme by his boss while traveling on business said before witnesses in TS-Si's presence
I thought I was going to be fired so I told him the first thing that came into my mind – that I really was transsexual. He bought it and I kept my job. The only problem is that he is so supportive that I have to dress like this 24-7 now if I want to have a job.
No man that I have ever met has expressed an actual physical desire to carry a baby within them for nine months. They may talk about it theoretically, when speculating on how it must feel, but when the rubber hits the road, no man wants nine months of discomfort that ends with the pain of labor.
 
The sexual binary is real. To be female is to belong to the sex that bears babies; to be male is to belong to the sex that that performs the fertilizing function in generation. Physical sex is not some disconnected academic gender theory. In the real world, women have and raise babies; men father them, and many times stay around to help. 
 
None of this, of course, addresses the decision to have bottom surgery or not. The cost is high, the technique still developing — that is a separate decision for HBS men. Money and poor health are legitimate reasons not to have an operation.
 
Wanting to become pregnant is not a legitimate reason for not having the operation. Pregnancy is a life choice but not one that a man would or could make. A transman who decides to become pregnant only plays into the hands of social conservatives who say HBS does not exist and we are really transvestites. An HBS man who chooses to retain her ability to give birth suggests that they are really a crossdresser or a transgender rather than a true HBS man.
 
If you want to have a child, adopt — there are plenty of children out there who need parents. If you insist that a child must be made from your own genetic material, pay for experimental surgery to merge two ova in vitro and pay someone to carry the fetus to term. There is no reason to love a child any less simply because you did not carry them to term.
 
Another reader posed a thought experiment to put things in perspective:
Consider the other side of the coin, a male to female "transsexual" who decides he is female, lives as a female full time but elects to keep his testicles and penis in working order. At a later time, he then impregnates "her" female partner.
 
Could a claim then be made that a Female was the Father of baby? Would Oprah rush to have the fully penised Female on her show?
 
Or would the media be talking about the male crossdresser who knocked up his wife? Would it then even be news? [N4]
Retention of the ovaries and uterus along with desire to become impregnated argues that Thomas Beattie, at her core identity, is a FEMALE. At best, she is transgendered or a crossdresser. She is not an HBS man.
 
I'm sorry but you can't have your cake and eat it too. Life forces you to make decisions. Hopefully we make informed ones.
 
Neither I nor TS-Si are tools of the right; nor are we a lockstep jackboot of the left. We subscribe to no particular political, social, or religious bent. If HBS men and women screw up in public and make the case for HBS more difficult for politicians and society to understand, we will say so.
 
Being HBS/transsexual does not give you a free pass from rationality or neurobiology.

If this person truly had always known that she was really a man, the idea of carry a child would never have entered her mind. The idea of keeping your fallopian tubes and ovaries because one day you may want to have a child is a woman’s thinking. I can’t think of any man who would want to carry a child to term.
