Female to Male (FtM) transition, when complete,has several steps. Surgical correction itself has three distinct components:
• Top Surgery or the removal of the female breast tissue. Pending a bilateral mastectomy, HBS men must bind their breasts to hide them. Membership on a “skins” basketball team is not an option.
• Internal surgery or the removal of the internal female organs (total hysterectomy, etc.). Failure to remove functioning ovaries throws the body into a conflict between the internally produced estrogen and externally injected testosterone.
• Bottom Surgery, or realignment of the urethra, concurrent with the construction of a penis (phalloplasty).
More to the point, what man wants to still have his monthly period or risk becoming pregnant? For practical reasons (the surgery techniques are less than perfect), many HBS men chose to have full hysterectomies without concurrent bottom surgery. Although not ideal, this seems a pragmatic compromise, making the best of current surgical state-of-practice.
Add to this the regular injection of testosterone to grow a beard, grow hair on the arms and legs, build muscles, deepen the voice, recede the hairline, and — if still present — enlarge the clitoris to some functioning equivalent of a smallish penis. This latter point is based on the testimony of HBS men. It should be understood in context, coming from men who describe their perceptions of their own penis size.

I have taken my time before writing this, mulled it over in my mind, read various newspaper articles, reviewed You Tube videos of this breastless vagina on Oprah, unable to decide if he is a fraud or she is a post-modern crazy. For those of you who have been watching college basketball for the last few weeks, Thomas Beatie was born female, underwent full breast removal (top surgery), and now lives as a male — except that he says he is pregnant. With Child. Knocked-up.
Enceinte. A bun in an oven that should be no longer functioning.
Rather than examine all the physical changes from woman to man to woman again, let’s accept them as true. A given. Instead, let’s look at the concept of male and female, using Thomas Beatie (fka Tracy Lagondino) as our science project — a fair course of action since Mr. Beatie has consciously made himself a public figure and fair game for the critical eye.
Given the state of the art, full sex reassignment surgery for Harry Benjamin Syndrome (HBS) men is more difficult than for HBS women [1]. It is a fact of life that it is more difficult to build or rebuild a working penis than it is to create or repair a vagina.
The first doctor who discovers how to make a functioning, erectible penis will gain renown but not necessarily riches. The first doctor who discovers how to repair a non-working penis will be a millionaire many times over as aging males lined up at his door to restore the vigors of their youth.
To build one or repair one, it isn’t easy. Many HBS men, rather than having a less than fully functional penis constructed, chose to do with out, satisfied with the physical effects of testosterone, the rush of being hormonally male, deep voiced, and well muscled.
I have not yet met a non-HBS male who wants functioning internal female organs: ovaries, uterus, and vagina with the concurrent cyclical menses. Males may fantasize about how it is to be female or imagine what it may be to pregnant, but few if any actually want the monthly bother, the PMS, or spending nine months carrying the developing fetus followed by actual vaginal delivery of the baby.
Although you might meet males who say they would like to try it, ask them, to verify their commitment to the cause so to speak, ask them if they are willing to have their penis cut off to do so. If you find one who doesn’t flinch at the collocation of penis and cut off, have them write me — they are probably HBS.
Now we come to the subject at hand, direct from his appearance on Oprah, Thomas Beatie, self-proclaimed woman into man … except HE still has functioning ovaries, a well stocked womb, and a desire to give birth.
I don’t want to put words into Tom’s mouth, so here are his own from the Oprah Winfrey Show during which he describes pregnancy as a "human desire":
I'm a person and I have the right to have my own biological child.
Translation: I want what I want when, where, and how I want it.
It doesn't make me want to go and shave my legs or something.
Translation: Hey, I don’t want you to think I’m some sort of fruitcake. I drink beer, scratch, and stare at women’s breasts.
I'm a man …
Translation: I’m really really really a real male …
I just happen to be a pregnant man.
Translation: I’m just a male with a working uterus who happens to be knocked up.
Beatie told Winfrey he had had a sex change but had not altered his reproductive organs because he wanted to have a child.
Translation: I didn’t really complete female to male sex realignment surgery because I wanted to remain female so I could carry a baby to term in my womb.
I think people will be shocked because no one's ever heard of a man giving birth before.
And we still haven’t. A person with functioning and fertile ovaries, with a functioning uterus, and a baby within her is female. Beattie may have no breast tissue, but neither does any woman who has had a double mastectomy.
This was my adult daughter’s reaction:
When I first heard about it, I thought it was a hoax. Then I heard that Oprah had him on and I knew that at least to some extent this had to be someone who had gone through with SRS. I watched a little bit of video on him and found out that when deciding to transition and have SRS she decided to keep her reproductive organs because "she may want to have kids one day."
I knew what we have was a very confused individual.
Men don’t think that way; men don’t think that they may want to carry a child in them. If this person truly had always known that she was really a man, the idea of carry a child would never have entered her mind. The idea of keeping your fallopian tubes and ovaries because one day you may want to have a child is a woman’s thinking. I can’t think of any man who would want to carry a child to term.
You almost have to feel sorry for her, to be that unhappy with herself, that confused. [2]
Thomas Beatie is not my daughter’s first encounter with an HBS man or woman (obviously). She is rather familiar with this columnist and other HBS men and women. So when something doesn’t feel right to her, we need to listen. My daughter has no horse in this race or ax to grind.
Beatie’s actions play into the social conservatives’ and religious rights’ assertion that HBS is lifestyle choice, not a birth condition. Beatie certainly seems to fluctuate between choices depending on his or her mood: today I shall be meat — tomorrow, fish. Beatie’s actions serve only to further confuse the issue of sex reassignment surgery in the public eye.
Beatie adds publicity, not clarity, to a very serious medical condition. I can only imagine the book deal or the Lifetime Movie that may be in the works.
Perhaps his fifteen minutes of fame is more important than her sexual identity.
Somehow Beatie has slipped through the protocols. What therapist affirmed that Beatie has a candidate for sex reassignment surgery (I would like to have her or his name so a proper inquiry could be conducted)?
Or did Beatie avoid therapy and find some willing, trendy surgeon eager to put sex reassignment surgery on his resume (although in truth, it was only a double mastectomy)?
Then there is the question of cost. Is Beatie a trust fund baby? Who is paying for all of this special medical treatment to support the pregnancy? Does Daddy know what Beatie is doing with his money? Beattie seems to have a lot of cash to throw around to pay for whatever his current desire might be.
Beatie is certainly very male in his need to fill his every desire.
I have the right …
The amount of testosterone that Beatie would have taken to masculinize his body should have shut his ovaries down, made them infertile. If the beard and voice are real, the ova should be dead.
Assume Beatie is who he says she is. He is a living lab experiment on the effects of lingering testosterone on the developing fetus. Despite the doctor’s assurances, we have no track record for the long term effects of the testosterone on the child. We can only hope.
Would a rational woman knowingly put the life of her baby in danger?
You have to come to your own decisions on that.
Me? I agree with my daughter: none of this feels right. As Shakespeare wrote in Hamlet:
